Top 3 PBMs Process 80% of Prescriptions in 2024: CVS Caremark, Express Scripts, and Optum Rx Lead the Market

Three pharmacy benefit managers (PBMs) continue to dominate the market, processing nearly 80% of all equivalent prescription claims in 2024. CVS Caremark (CVS Health), Express Scripts (Cigna), and Optum Rx (UnitedHealth Group) remain the major players, although shifting relationships and contract changes have led to some realignment within the industry. Express Scripts has taken on a more prominent role. According to data from Drug Channels Institute (DCI), these three familiar companies maintain a strong hold on the PBM landscape. While the names remain the same, the dynamics between them have evolved due to contract shakeups. A webinar further discussing these trends, challenges, and future outlooks is planned for April 4, 2025.
