Trump Administration’s Halt on Federal Health Agency Communications Raises Alarming Transparency Risks

The Trump administration has temporarily halted public communications from several federal health agencies until at least the end of the month. According to a memo acquired by The Associated Press, acting U.S. Department of Health and Human Services (HHS) Secretary Dorothy Fink informed agency staff leaders on Tuesday of an “immediate pause” on regulations, guidance, announcements, press releases, social media content, and website updates. These communications now require approval from a political appointee before release.

The directive has raised concerns about transparency and the flow of critical information from federal health agencies to the public. The freeze also extends to materials intended for publication in the Federal Register, which is used by the executive branch to communicate rules and regulations, as well as the Centers for Disease Control and Prevention’s Morbidity and Mortality Weekly Report, a scientific publication. The memo stated that the pause will remain in effect until February 1.

The mandate applies to agencies under the HHS, including the CDC, the National Institutes of Health, and the Food and Drug Administration, all of which are organizations responsible for managing epidemics, ensuring food safety, and advancing disease research. Four federal health officials confirmed the pause, speaking anonymously because they were not authorized to discuss the matter. US’s HHS officials have not commented on the communication freeze.

A former HHS official stated on Wednesday that it is not uncommon for incoming administrations to pause agency communications for review. However, transition teams usually ensure that the document issuance process runs smoothly by inauguration day. “The executive branch is a hierarchy,” said Steven Grossman, now a consultant for food and drug companies, in an email. “Whether stated publicly or not, every new administration wants important commitments and positions to wait until new teams are in place and some semblance of hierarchy restored.”

During Trump’s first term, his political appointees pushed to take more control over the CDC’s MMWR journal, which had shared COVID-19 updates that sometimes clashed with the White House’s messaging. HHS Secretary, Fink, mentioned that some communications could still go out if they addressed “critical health, safety, environmental, financial, or national security functions,” but even those would need approval. Despite the freeze, the FDA managed to post some updates on Tuesday and Wednesday, including warning letters to companies and a drug safety notice.

Peter Lurie, MD, MPH, president of the Center for Science in the Public Interest, highlighted the importance of timely updates from agencies like the CDC and FDA in preventing foodborne illnesses and addressing health concerns. He warned that the communication freeze could cause delays in issuing urgent public health alerts, stating, “When it comes to stopping outbreaks, every second counts.” Lurie also noted that the vague wording of the directive could create confusion and hinder effective responses during active outbreaks.

Jeffrey Klausner, MD, MPH, a public health expert at the University of Southern California, shared similar concerns. He emphasized that local health officials and doctors rely on the CDC for disease updates, prevention strategies, and outbreak information. “Shutting down public health communication stops a basic function of public health,” Klausner said, adding that it is comparable to “turning off fire sirens or other warning systems” during emergencies.
